Blood pressure medications, prescribed to approximately nine million people annually in the United Kingdom, play a critical role in reducing the risk of heart attacks and strokes. Despite their widespread use and proven benefits, research indicates that nearly half of patients do not adhere consistently to their daily regimens, with about 20% discontinuing treatment altogether. This noncompliance is attributed to a combination of forgetfulness, especially among older adults, and adverse side effects that some patients find difficult to tolerate.
Common complaints from patients taking these drugs include swollen ankles, chronic coughing, increased urination, and gastrointestinal issues, which in some cases have led individuals to cease medication without consulting healthcare providers. A recent study from Israel raised additional concerns by linking a subgroup of blood pressure drugs—calcium-channel blockers—to an elevated risk of kidney disease. Nonetheless, medical experts emphasize that these medications are generally safe and remain essential for long-term cardiovascular health.
“High blood pressure is often symptomless, so patients may not feel immediate benefits from the medications, which can deter adherence,” said Professor Amitava Banerjee, a cardiology specialist at University College London. “However, over time, these drugs significantly reduce the likelihood of fatal cardiac events.”
Blood pressure measures the force exerted by circulating blood on arterial walls, fluctuating naturally throughout the day. Persistent high blood pressure, known as hypertension, places strain on blood vessels and increases the risk of heart attacks and strokes by promoting arterial damage and the buildup of fatty deposits. A normal blood pressure reading is around 120/80 mmHg, while values consistently above 140/90 mmHg indicate hypertension.
The prevalence of high blood pressure in the UK is on the rise, currently affecting roughly one-third of adults, with many remaining undiagnosed. Experts describe hypertension as a leading contributor to mortality, underscoring the importance of effective treatment.
Several classes of antihypertensive drugs are available, each with distinct mechanisms and potential side effects. ACE inhibitors, such as ramipril and lisinopril, can cause dizziness and persistent dry coughs, while calcium-channel blockers like amlodipine are commonly associated with headaches, facial flushing, constipation, and ankle swelling. Diuretics, including indapamide, increase urine production but may lead to frequent urination that some patients find burdensome.
For patients experiencing intolerable side effects, healthcare professionals often recommend adjusting dosages or switching to alternative medications. Additionally, lifestyle modifications can be effective both as complementary measures and as potential pathways to reducing or discontinuing drug therapy.
“While some patients will require lifelong medication, others may achieve healthy blood pressure levels through changes such as weight loss and dietary adjustments,” Professor Banerjee said.
Reducing body weight has been linked to lower blood pressure, as excess weight increases cardiac workload. A 2013 study from the United States found that shedding 10% of body weight enabled many participants to eliminate the need for at least one blood pressure drug, although such weight loss typically takes months to achieve.
In contrast, lowering dietary salt intake can lead to significant blood pressure reductions within weeks. The majority of salt intake in the UK comes from processed foods and ready meals rather than from salt added during cooking or at the table. Cardiac nurse June Davison from the British Heart Foundation highlighted the importance of cutting back on high-salt convenience items like frozen meals and processed meats to achieve health benefits.
“Adopting a low-salt diet overall is crucial. Once this is achieved, a small amount of added salt at mealtime is less of a concern,” Davison explained.
Overall, healthcare providers encourage patients managing hypertension to discuss any side effects or adherence challenges, and to consider lifestyle interventions alongside medication for optimal outcomes.
