Recent discussion on hormone replacement therapy (HRT) has renewed focus on its potential benefits for women, particularly in relation to dementia risk and overall health during and after menopause. This follows a report suggesting that HRT may reduce the likelihood of developing dementia among women.

Michael Baum, professor emeritus of surgery and visiting professor of medical humanities at University College London, highlighted the evolutionary perspective that women’s biological usefulness was traditionally viewed in terms of fertility. He noted that in Britain around 1900, the average life expectancy for women was approximately 50 years, roughly the end of their fertile period. In contrast, modern women live significantly longer — often three to four years longer than men on average — leaving many years to live without the natural hormone levels that offer protection against conditions such as osteoporosis.

Baum suggested that this extended postmenopausal lifespan means women experience decades of hormone deficiency, which can contribute to various health issues. He drew a comparison to thyroid hormone replacement, where no hesitation exists in prescribing medication to replace missing thyroxin. By contrast, he stated that many women undergoing menopause have been denied estrogen replacement due to concerns about increased breast cancer risk.

Addressing this controversy, Baum argued that the notion of estrogen increasing breast cancer risk is not supported by current evidence, and in fact, estrogen may offer some protective effects. He asserted that the benefits of prescribing HRT after age 50 now appear to outweigh the potential harms, especially considering emerging data on cognitive health and dementia prevention.

This perspective adds to ongoing debates in medical communities and among patients regarding the safety and efficacy of HRT. While some healthcare professionals remain cautious due to historical studies linking hormone therapy and cancer risk, others emphasize newer research indicating possible advantages for long-term health and cognitive function.

As the dialogue continues, experts suggest that individualized approaches are important in determining HRT use. The balancing of benefits, such as improved bone health and potential dementia risk reduction, against any potential adverse outcomes remains central to clinical decisions about hormone therapy in postmenopausal women.
