A new medication taken nightly has been shown to significantly reduce breathing interruptions in patients with obstructive sleep apnea (OSA), according to recent research. The condition affects an estimated eight million people in the United Kingdom and is characterized by repeated pauses in breathing during sleep, often accompanied by loud snoring, gasping, or snorting. Daytime symptoms frequently include excessive sleepiness and fatigue, while long-term effects can increase the risk of serious cardiovascular and metabolic diseases.
Currently, continuous positive airway pressure (CPAP) machines are the standard treatment. These devices deliver mild air pressure via a mask to keep the airway open throughout the night. However, many patients find CPAP uncomfortable or intolerable, leading to non-compliance or refusal; nearly half of those prescribed a CPAP machine do not use it consistently or at all.
Researchers from the University of Pittsburgh Medical Center conducted a large-scale trial to evaluate an oral alternative, known as AD109. This investigational drug combines two existing medications, aroxybutynin and atomoxetine, aimed at strengthening throat muscles and preventing airway collapse during sleep. The study involved 646 adults with varying degrees of sleep apnea severity who were either unwilling or unable to use CPAP devices. Conducted over six months at more than 60 sites across the United States and Canada, the trial found that patients taking AD109 experienced a 44 percent reduction in the number of breathing interruptions per hour on average. Other nocturnal symptoms also showed improvement.
The results were consistent among individuals with a wide range of disease severity and body types. Lead author Dr. Patrick John Strollo noted that, unlike other chronic diseases where untreated or undertreated patients are rare, a substantial proportion of those with OSA remain without adequate therapy. He suggested that an effective oral treatment targeting the neuromuscular causes of airway collapse could help close this treatment gap.
These findings represent a potential advance in managing sleep apnea, particularly for patients who have difficulties with existing therapies. Further studies may be needed to assess long-term efficacy and safety before the drug becomes widely available.
