Optum Rx announced a new initiative aimed at increasing transparency and reducing complexity within the pharmacy benefit management (PBM) industry. The company is shifting to a fully transparent, fee-based pharmacy care model that decouples its earnings from drug prices and prescription volumes, a significant change designed to offer clients clearer insight into costs and fees.

The current system, characterized by intricate layers of pricing and rebate mechanisms, has made it difficult for employers to control expenses and for consumers to understand and afford their medications. Drug manufacturers’ rising prices have placed increasing financial pressure on patients, while critics have long argued that the PBM industry benefits from maintaining opacity around pricing and rebates.

In response, Optum Rx plans to provide full disclosure of fees charged to clients, including payments received through group purchasing organizations (GPOs). By the end of 2027, the company intends to move to flat service fees for group purchasing, simplifying pricing structures and making costs more predictable. The company is also advancing its commitment to pass through 100% of manufacturer rebates to its clients by 2028; currently, 98% are already fully passed through.

The changes are intended not only to help employers better manage pharmacy benefit costs but also to improve the consumer experience. Optum Rx has introduced digital tools such as Shop MyScript, which offers patients real-time pricing, pharmacy options, and delivery methods immediately after a prescription is issued. Another tool, Price Wise, provides a transparent, all-inclusive cost breakdown—including drug price and service fees—allowing consumers to avoid unexpected charges at the pharmacy counter. Additionally, the Price Edge program has reportedly saved members $273 million by ensuring access to the lowest available medication prices.

Industry observers note that these developments reflect a broader trend toward transparency and accountability in pharmacy care. As transparency becomes a standard, competition is expected to focus more on delivering value, service quality, and health outcomes rather than on navigating complex pricing models.

Optum Rx acknowledges that transforming the pharmacy benefit system will take time but emphasizes that these steps mark a move toward rebuilding trust with employers, policymakers, and consumers. The company asserts that transparency is vital to fostering a more predictable and accountable pharmacy benefit management industry aligned with the needs of its stakeholders.
