A growing public health concern has emerged in parts of the United Kingdom with the rise of ketamine adulterated with medetomidine, a potent veterinary tranquiliser. This new contaminant, often sold on the streets as "Rhino Ket" or "Rhino Tranq," has significantly increased the risks associated with the drug, causing severe sedation, paralysis, and making users unusually vulnerable to harm, including sexual assault.
Medetomidine, used primarily on large animals in veterinary medicine, is reportedly 200 times more powerful and longer lasting than xylazine—a sedative previously known to be mixed with ketamine. Its presence in street drugs has led to alarming incidents, especially among young users aged 16 to 24, a demographic in which ketamine use has more than tripled since 2016.
University of Manchester researchers and local drug services have confirmed multiple samples of ketamine contaminated with medetomidine, issuing warnings about its extreme sedative effects and the heightened potential for respiratory problems, overdose, and death. Wirral Ways, a drug and alcohol support organisation, has similarly cautioned that this mixture may cause profound sedation and breathing difficulties.
Individual accounts highlight the stark dangers. A nurse, who asked to be identified only as Becky, described being conscious but entirely paralysed after consuming a tainted batch, during which she was sexually assaulted. Despite recognizing the danger, fear of losing her job prevented her from reporting the attack. Her experience underscores the drug's ability to incapacitate users while leaving mental awareness intact, rendering them unable to protect themselves.
Another user, Mark, a 20-year-old trainee accountant from Greater Manchester, recounted collapsing and crashing his car after ingesting the contaminated drug. Avoiding police intervention due to career concerns, he narrowly escaped fatal consequences. His case illustrates how professionals from various backgrounds are increasingly exposed to this drug, which has infiltrated social circles that traditionally shunned heavy drinking in favor of recreational drugs perceived as less impairing.
Addiction counsellors in Lancashire report a rapid rise in confirmed cases linked to medetomidine-adulterated ketamine since late 2025. Therapist Steve Pope noted an increase from virtually no cases to more than 60 in his area alone, with the spread accelerated recently by social events such as the World Cup. The trend spans diverse communities, including teenagers with no prior drug experience and professionals such as accountants, lawyers, footballers, medical students, armed forces personnel, and healthcare workers.
The drug market in cities like Manchester and Blackpool is increasingly dominated by syndicates from Eastern Europe, with reports of violent repercussions for families unable to settle debts related to drug addiction. Medetomidine’s presence is particularly concerning because it is not a narcotic and its effects cannot be reversed by naloxone, the opioid overdose antidote commonly used by emergency responders. Regulatory efforts have included the UK's government moving to classify medetomidine as a Class C controlled substance in early 2026. However, experts argue this designation may be insufficient to deter dealers, who see it as relatively low risk and attractive to recreational users.
Authorities and health services continue to advise drug users to test ketamine before consumption using specialized kits that detect medetomidine contamination. Nevertheless, the cost and accessibility of these tests limit their use, especially among young people purchasing low-cost supplies.
