A 34-year-old patient was fatally attacked by a fellow patient at an acute mental health ward in east London, an inquest has revealed. The incident took place on January 3, 2023, at the Newham Mental Health Centre’s inpatient ward for men.

Hugo Flint-Cahan was assaulted and strangled by 22-year-old Rolando Torres-Pena. According to testimony, three unstable patients were left unsupervised in the ward’s corridors while one nursing assistant was asleep and two nurses remained behind a closed staff-room door during the attack.

Jolyon Flint-Cahan, an NHS doctor and brother of the deceased, expressed deep frustration over the circumstances leading to the fatal incident. He stated that both families involved and Hugo himself were “utterly failed by the staff on duty” that night. He emphasized the vulnerability of individuals in mental health care settings and highlighted the ethical responsibility staff have to ensure patient safety.

The senior coroner for east London, Graeme Irvine, concluded that Hugo Flint-Cahan was unlawfully killed, with neglect identified as a significant contributing factor. Irvine recommended that four members of the ward staff be referred to their professional regulator and called for the Metropolitan Police to review the investigation into the incident.

During the inquest, Irvine remarked on the recurrence of similar failings in mental health settings, describing the case as “ground hog day,” noting that he had repeatedly encountered evidence of “the same” errors leading to patient harm.

The findings have reignited concerns about staffing levels, supervision, and safety protocols in acute mental health environments, underscoring the challenges of managing vulnerable patients in inpatient facilities. Authorities have yet to announce any further actions following the coroner’s recommendations.
