A recent randomized controlled trial conducted by the Italian National Institute of Gastroenterology has provided new insights into the potential health benefits of tomato consumption for individuals with fatty liver disease. The study, published in the journal Nutrients, investigated how daily intake of tomatoes affected liver fat accumulation over a six-week period.

The trial enrolled 79 participants diagnosed with fatty liver disease, a condition linked to obesity, insulin resistance, and type 2 diabetes, which can progress to inflammation, scarring, cirrhosis, and liver cancer. Participants were divided into two groups: one consumed 200 grams of raw tomatoes alongside 50 grams of tomato sauce daily, while the other group avoided tomatoes entirely.

Liver fat content was measured using FibroScan technology, specifically assessing the controlled attenuation parameter (CAP), a metric indicating the severity of fat deposition in the liver. Results showed that the tomato-consuming group experienced a reduction of 35 points in CAP scores, compared to an 18-point decrease in the group avoiding tomatoes. The modest decline in liver fat among the non-tomato group may reflect lifestyle changes prompted by study participation.

Notably, the improvements in liver fat were not accompanied by significant changes in participants’ body weight, body mass index (BMI), waist circumference, total body fat, or visceral fat. Additionally, standard blood markers including cholesterol, blood glucose, and liver function tests remained similar between groups. These findings suggest that the observed benefits were not simply due to weight loss but may reflect a more direct effect of tomato compounds on liver metabolism.

Researchers hypothesize that bioactive components in tomatoes, particularly lycopene and beta-carotene, could influence hepatic fat management by reducing fat synthesis, limiting fat uptake, and promoting fat oxidation in the liver. While the study sample was relatively small and short-term, the findings align with broader evidence associating tomato consumption with positive health outcomes.

For instance, a large observational study of over 100,000 US adults followed for an average of 17 years found moderate tomato intake was linked to lower overall mortality and reduced risk of cardiovascular disease. Similarly, a meta-analysis of 119 studies indicated that higher tomato consumption was correlated with an 11 percent reduced risk of cancer-related deaths, with some data suggesting specific protection against prostate cancer. However, these associations remain observational, and causality has yet to be established.

Experts caution against reliance on lycopene supplements, pointing out that current evidence primarily supports consuming whole tomatoes, which provide a complex mixture of antioxidants, polyphenols, and vitamins. The Italian researchers emphasized the complementary benefits of both raw and cooked tomatoes, noting that cooking enhances lycopene absorption, particularly when combined with dietary fats such as olive oil, while raw tomatoes preserve vitamin C content.

Given the increasing prevalence of fatty liver disease and its associated health risks, incorporating tomatoes into the diet may offer a safe, accessible, and inexpensive means to support liver health. Further research is needed to confirm these effects and explore long-term outcomes, but current findings suggest that dietary strategies focusing on whole foods like tomatoes could play a role in managing and potentially preventing fatty liver disease.

Separately, emerging pharmacological approaches targeting obesity and metabolic health are in development, including drugs modeled on exercise-induced compounds that may preserve muscle mass during weight loss. However, maintaining muscle health through resistance and high-intensity exercise remains a cornerstone of metabolic wellness.
