A small town in western Uganda is maintaining vigilance as a deadly Ebola outbreak continues to affect neighboring Democratic Republic of Congo (DRC), highlighting the challenges of managing cross-border public health emergencies.

Bundibugyo, located near the Lamia River which forms part of Uganda’s more than 800-kilometer border with eastern Congo, has enacted strict border closures since May, shortly after the World Health Organization (WHO) announced a new outbreak in the DRC. Until recently, the bridge over the river served as a vital link for local farmers and schoolchildren who crossed daily into Congo. However, Ugandan authorities have now restricted movement, with soldiers stationed at the border signaling that crossings are halted.

The current Ebola outbreak in eastern Congo is caused by the Bundibugyo virus strain, first identified in this Ugandan town in 2007. This virus subtype is rare and has complicated initial detection efforts for health officials, who initially tested for more common Ebola strains before identifying the Bundibugyo variant as the cause of the outbreak. The WHO reports that the epidemic in Congo has killed over 1,300 people and infected at least 3,000, though they caution that actual figures may be significantly higher.

Uganda has recorded 20 confirmed cases linked to this outbreak, mostly involving individuals crossing from Congo to seek treatment. Authorities report that no new cases have emerged in over a month, and Bundibugyo itself has not documented any local infections during this episode. Fifteen patients treated in Uganda have mostly recovered; two fatalities were reported, while others were quarantined and subsequently released.

Residents of Bundibugyo have deep experience with Ebola, drawn from the initial outbreak nearly two decades ago, when the virus was only recognized weeks into the crisis. Local healthcare workers suffered greatly at that time, including nurse Ezekiel Kisughu, who contracted the disease while treating a critically ill patient. He survived but recounted the traumatic stigma attached to infection and the lack of protective measures when the virus first emerged.

Since then, Uganda has developed a strong reputation for efficiently managing infectious diseases such as HIV, COVID-19, and Ebola. Bundibugyo’s healthcare workers, supported by national and international health agencies, routinely update protocols and remain prepared. Public life in the town has largely continued uninterrupted, with schools, shops, and government offices open and hand-washing stations installed at public entrances. Mask-wearing is not reported as widespread within the community.

Despite these precautions, the border closure has disrupted local economic activities. Many Bundibugyo residents have historically relied on cross-border trade and agriculture, with farming on the Congo side offering cheaper land. The shutdown has strained families, particularly with some parents unable to pay school fees. The head teacher of a local primary school noted the hardship faced by families and described efforts to educate children on recognizing Ebola symptoms and hygiene practices.

Long-term shifts in community habits have also occurred since the first outbreak, including a move from subsistence farming to cocoa production, which dominates the local landscape. Residents report reduced consumption of wild animals—such as bats and monkeys—that may serve as reservoirs for Ebola, influenced also by the creation of nearby national parks restricting hunting.

Experts emphasize the porous and complex nature of the Uganda-Congo border, underscoring the intertwined economic and social relations that complicate efforts to prevent disease spread. Ugandan health officials acknowledge that maintaining control over Ebola risks in such a context remains challenging. While border closures aim to prevent cross-border transmission, some international public health experts warn these measures can hinder transparency and cooperation during outbreaks.

For now, Bundibugyo remains alert but stable as Uganda continues to monitor the evolving situation just across the river.
