BALTIMORE — The opioid landscape in Baltimore has shifted dramatically in recent years, with synthetic drugs now dominating the street market and driving a surge in overdose deaths. Local residents with extensive experience on the city’s drug trade describe an unpredictable supply chain marked by potent, mixed chemicals and rapidly changing substances.
Since the early 2000s, Baltimore has seen overdose mortality rates climb sharply, with recent estimates reporting 110 deaths per 100,000 residents—among the highest rates in large U.S. cities. The crisis began with prescription opioids, progressed to heroin, and in the last decade has been largely fueled by synthetic opioids, particularly fentanyl. Now, a growing array of lab-produced compounds, often blended with other sedatives and narcotics, contribute to a more dangerous and less identifiable drug supply.
Individuals formerly involved in Baltimore’s drug scene—including a man known as Sleepy, 44, and a woman called Boog, 40—highlight how synthetic opioids have transformed the economics of the trade. Both have histories in drug sales and incarceration. They report that heroin, as previously known, has virtually disappeared from the city’s streets, replaced almost entirely by fentanyl and other synthetic analogs. Dealers add various substances to increase potency and extend quantities, a practice that makes products cheaper to produce and more profitable to sell.
“The profit is bigger because it’s cheaper, and you get to add more, different chemicals and cut it to make more money,” Sleepy explained, noting a shift from bulkier, farm-grown drugs to compact, chemically synthesized ones manufactured in clandestine labs. Reports indicate that much of the fentanyl supply originates in Mexico, where precursor chemicals are sourced from factories in China, allowing fentanyl to be produced at very low cost.
The rising potency and lower cost mean users can purchase stronger doses for less money than in the past—a factor experts associate with increasing overdose fatalities. While city homicide rates linked to gangs have declined since 2019, the drug trade remains robust, fueled by more affordable and potent synthetic opioids.
Sleepy and Boog described a street ecosystem adapting to volatility, including the use of “testers” who sample batches for quality and potency before drugs reach general users. One long-time user and seller, known as Pops, serves this role, identifying dangerously strong or unusual mixtures amid the proliferation of compounds. The expanded variety and unpredictable drug combinations have led to increased overdoses, including incidents last summer involving fentanyl mixed with N-methylclonazepam, a potent sedative.
Users themselves face daily uncertainty about what they consume. A man identified as Berto, 25, described the indistinct and often perilous nature of the street supply. Although “heroin” remains a common term, the actual drug is almost always fentanyl or similarly powerful synthetics, often “scrambled” with multiple additives. Berto recounted experiencing intense withdrawal symptoms and suffering multiple overdoses, some requiring Narcan administration to reverse.
Some dealers reportedly market highly potent capsules, nicknamed “death packs,” which may cause overdoses but paradoxically attract customers because of their strength. This dynamic underscores the lethal risks facing users in an environment where drugs grow increasingly potent and chemically complex.
The transformation in Baltimore’s drug market—from plant-derived substances to lab-engineered compounds—has reshaped the city's addiction and overdose landscape. Despite law enforcement efforts and harm reduction strategies, the arrival of ultrapotent synthetic opioids continues to pose critical challenges for public health and community safety.
