An expert panel convened by the American Heart Association (AHA) has determined that consuming up to 400 milligrams of caffeine daily—equivalent to three to five eight-ounce cups of coffee—is generally safe for most adults and may offer cardiovascular benefits. The panel’s conclusions are based on a comprehensive review of decades of research examining caffeine’s effects on heart health, including studies on blood pressure, cholesterol, coronary heart disease, stroke, heart failure, atrial fibrillation, and type 2 diabetes.
Published July 20 in the AHA’s journal Circulation, the scientific statement draws from a wide array of observational studies, clinical trials, genetic research, and recent randomized controlled trials. The findings suggest that moderate coffee consumption is associated with a reduced risk of several cardiovascular conditions as well as type 2 diabetes.
Although caffeine can cause a temporary increase in blood pressure, evidence indicates that regular coffee drinkers develop tolerance, mitigating this effect over time. Long-term studies have consistently shown either no increase in hypertension risk or a lower risk associated with moderate coffee intake. The research also highlights strong protective associations between habitual coffee drinking and reduced incidence of type 2 diabetes; a review of 28 observational studies found a 20 to 30 percent lower risk of developing diabetes among habitual consumers of approximately 3.5 to 5 cups per day.
Regarding atrial fibrillation, most studies reported no increased risk linked to caffeine consumption, and several pointed to potential risk reduction. A recent controlled trial found a 39 percent lower recurrence rate of atrial fibrillation among patients consuming one cup of caffeinated coffee daily compared with those who avoided caffeine entirely.
The panel cautioned, however, that not all coffee preparations should be considered equally. Unfiltered coffee types such as French press, Greek or Turkish coffee, and Scandinavian boiled coffee contain cafestol—a compound that has been shown to raise low-density lipoprotein (LDL) cholesterol levels. Filtered and instant coffees contain minimal cafestol and are less likely to have this effect.
The authors also underscored that the health effects observed with coffee may not apply to other caffeine sources, particularly energy drinks. These often contain higher caffeine concentrations—three to four times that of coffee—and may adversely affect cardiovascular health by raising blood pressure and triggering abnormal heart rhythms. Additionally, energy drinks and caffeine supplements can include other ingredients that modify caffeine’s absorption and impact.
Beyond caffeine, coffee contains numerous bioactive compounds, such as polyphenols and chlorogenic acid, which may contribute to its health benefits by reducing oxidative stress, inflammation, and improving glucose metabolism. Coffee also provides trace amounts of minerals like magnesium and chromium, which play roles in insulin function.
The AHA’s findings align with Health Canada’s existing guideline recommending a maximum daily caffeine intake of 400 milligrams for most adults, while noting a lower limit of 300 milligrams for pregnant or breastfeeding women. Both emphasize that individual tolerance to caffeine varies widely based on factors including age, genetics, medications, and health status. Adverse effects such as sleep disturbances, palpitations, headaches, and anxiety may occur if consumption exceeds an individual’s tolerance.
